Honestly, I think it is a bogus concern. Here's what I could find in the medical literature:

OBJECTIVE: The purpose of this study was to clarify the influence of automobile driving in the third trimester of pregnancy on fetal heart rate and uterine contraction. METHODS: Twenty-nine pregnant women with singleton pregnancies after 28 weeks of gestation were monitored using a portable cardiotocogram during driving, and the results obtained before and after driving were compared. RESULTS: Baseline fetal heart rate and its variability, as well as periodic fetal heart rate patterns, showed no abnormal patterns during car driving. Automobile driving in late pregnancy was not a reinforcement factor for uterine activity. Maternal systolic and diastolic blood pressure and heart rate showed no specific characteristic or adverse change during driving. CONCLUSION: The results of this study suggest that automobile driving in the third trimester of pregnancy has no adverse influences on either pregnant women or fetuses. -------------------------------------

The physiologic reactions of pregnant women and their fetuses were studied during routine commercial flights. Ten healthy pregnant women (32 to 38 weeks of gestation) each undertook two flights. Maternal respiratory and heart rates, transcutaneous PO2 and PCO2, blood pressure, uterine activity, and fetal beat-to-beat heart rate variability were continuously monitored. During these flights, maternal heart rate and blood pressure increased, and PO2 decreased significantly while PCO2 remained unchanged. Respiratory rate showed a short increase during takeoff and landing but remained unchanged during the rest of the flight. Mean fetal heart rate was within normal limits during the whole flight. No bradycardia, prolonged tachycardia, or significant loss of heart rate variability was observed. This study indicates no hazards of commercial flights to the mother and the fetus in uncomplicated pregnancies. ---------------------------------------

I could not find *ANY* studies suggesting that driving or riding in a car during any trimester was in any way unsafe. I went on a long car trip while 20 weeks pregnant with my first with no untoward effects. I wouldn't worry about it one bit. I would simply plan lots of stops, which you'd have to do even without the thrombosis risk because at 20 weeks she's going to get uncomfortable more quickly and need to stop for bathroom breaks more frequently anyway. Come to think of it, I'm sure I did more than 2 hours per day in a car many, many times through all three pregnancies with nary a problem.
