If I were to discreetly point out a melanoma and state that that that's an outcome of over-exposure to sun, would you object that it's "about appearance"??

This is just nuts. Obesity is visible. It just plain is. That's the visible presentation of the pathology. What do you want of people, to say about it?
I'm not saying "hey he's ugly dont' get like that". I'm saying that's a BAD way for a body to be for health reasons.
I dont' point to strangers on the street. But, frankly, I know of *no* obese person about the age of about 30 who hasn't developed health problems due to it.
I don't completely agree, but hopefully you can see in what you've written is the reason why I can't present obesity as a normal thing that "just happens".
